Antonette Mentor
Antonette Mentor, LCSW, ‘01 is a graduate of CSSW Advanced Generalist Practice and Programming Method with a specialization in Aging, and where she was awarded a Geriatric Fellowship by the Hartford Foundation. Her undergraduate degree in Psychology was completed at the University of the Philippines (Diliman) and Brooklyn College, CUNY. Antonette is pursuing a Doctorate in Public Health at Johns Hopkins Bloomberg School of Public Health, specializing in Health Equity and Social Justice.
Antonette’s Social Work career began in Kings County Hospital Psychiatric Department where she trained as an in-patient social worker and primary therapist for adolescents and families. Her expertise in the aging population was developed starting with a practicum placement at the New Jewish Home, followed by several leadership roles in quality improvement and program management. She is currently an Associate VP at Montefiore, CMO, where she leads a number of population health management strategies. Her practice is at the intersection of Physical Health, Behavioral Health, Social Service and Payer Systems. She is the Executive Director of Montefiore Health Home, a New York State Plan Benefit that provides care management to nearly 8,000 Medicaid beneficiaries in the Bronx. Her business portfolio also includes the executive leadership of health plan-delegated Care Management (CM) and Utilization Management (UM) functions for members of Medicare, Medicaid, Commercial and ERISA plans. She leads the NCQA Accreditation of Montefiore CMO in UM, (Complex) CM and (Provider) Credentialing. She is the Executive Project Director in the implementation of the Integrated Care for Kids Program, a Centers for Medicare and Medicaid Innovation grant, to nearly 50,000 beneficiaries in the Bronx. She oversees the operation of a start-up Telemental Health practice with nearly 200 patient panel. Lastly, she oversees care transition initiatives in the ambulatory, ER and inpatient settings that are designed to avoid repeat ER admissions and inappropriate inpatient admissions.
Antonette calls New York and New Jersey home, where she resides with her husband, two daughters and large family of origin.
