Admitted Students Checklist

You’ve made it! Now what? To ensure your enrollment process goes smoothly, please complete the following tasks by the dates listed:

1. Complete the Free Application for Federal Student Aid (FAFSA) to apply for financial aid.
 2. Submit the Field Education Placement Form (Two Year, 16-Month, Advanced Standing, or Transfer Programs only; Columbia e-mail account and a PDF or Microsoft Word copy of your resumé are required). Note: The Field Education Department requires this form of all incoming full-time students for capturing key information for use in determining your field placement.
 3. Students who confirm their enrollment, will receive an email from the Office of Student Services to submit the registration form.
 4. Activate your University Network ID (UNI) and Columbia University e-mail. Your UNI will not be created until your tuition deposit has been received.
 5. International Students Only: Apply for a student visa.
 6. Statistics Requirement: Decide whether you will opt to take the statistics waivers exam before the beginning of first term to demonstrate your proficiency in statistics. Successful completion of the statistics waiver exam exempts a student from taking the course during the first semester of enrollment. Additional information about the School’s statistics requirement and course can be found here.
 7. If you have not already done so, send in your final, official transcript(s) showing undergraduate degree received and date awarded to Columbia School of Social Work, Admissions Office, 1255 Amsterdam Avenue, New York, NY 10027.
 8. Submit a photo for your CU ID card (UNI & PW required).
9. Submit immunizations documentation for measles, mumps and rubella (MMR Form). NOTE: You will not be able to register for classes without completing the New York State immunization requirements.
10. Dual Degree Students Only: Mail or e-mail your acceptance letter from the partner school to Columbia School of Social Work, Office of Student Services, 1255 Amsterdam Avenue, New York, NY 10027.
11. To receive disability accommodations, register with the Office of Disability Services
12. Apply for transfer credit.
13. If your enrollment plans have changed and you wish to defer your start term, please complete this form. Admitted students may request a one-time deferral of their admission only after submitting the nonrefundable enrollment deposit. Students may request either the next available Fall or Spring terms. Multiple deferrals are not permitted. Deferrals are approved at the discretion of the Admissions Office. If you were admitted for the Fall semester, the deadline to defer your start term is August 31st. If you have been admitted for the Spring semester, the deadline to defer your start term is December 31st. Any requests received after these dates will not be approved and you must reapply for your desired term.
