You’ve made it! Now what? To ensure your enrollment process goes smoothly, please complete the following tasks by the dates listed:

1. Complete the Free Application for Federal Student Aid (FAFSA) to apply for financial aid. ASAP
 2. Submit the Field Education Placement Form (full-time students only; Columbia e-mail account and a PDF or Microsoft Word copy of your resumé are required). Note: The Field Education Department requires this form of all incoming full-time students for capturing key information for use in determining your field placement. ASAP
 3. Submit the Registration Form: 16-month ProgramTwo-Year Program | Extended and Reduced Residency Program | Advanced Standing Program | Transfer Program |  | Online Campus | 5-term International Student Program   The submission of this form confirms your plan to enroll at the School and informs the Office of Student Services of pertinent information so they can register you for classes. ASAP
 4. Activate your University Network ID (UNI) and Columbia University e-mail. Your UNI will not be created until your tuition deposit has been received. ASAP
 5. International Students Only: Apply for a student visa. ASAP
 6. Submit immunizations documentation for measles, mumps and rubella (MMR Form). NOTE: You will not be able to register for classes without completing the New York State immunization requirements. June 1, 2016
 7. If you have not already done so, send in your final, official transcript(s) showing undergraduate degree received and date awarded to Columbia School of Social Work, Admissions Office, 1255 Amsterdam Avenue, New York, NY 10027. June 1, 2016
 8. Submit a photo for your CU ID card (UNI & PW required). June 1, 2016
9. Statistics Requirement: Decide whether you will opt to take the statistics waivers exam before the beginning of first term to demonstrate your proficiency in statistics. Successful completion of the statistics waiver exam exempts a student from taking the course during the first semester of enrollment. Additional information about the School’s statistics requirement and course can be found here. ASAP
10. Dual Degree Students Only: Mail or e-mail your acceptance letter from the partner school to Columbia School of Social Work, Office of Student Services, 1255 Amsterdam Avenue, New York, NY 10027. June 1, 2016
11. To receive disability accommodations, register with the Office of Disability Services June 1, 2016
12. If you register for any online courses, read this Online Student Agreement (PDF: 1 page), sign it, and submit via email to June 30, 2016
13. Apply for transfer credit. June 30, 2016
